This thesis investigates the pharmacokinetics (PK) of anti-PD-1 drugs and their relationship with treatment outcomes in oncology. Monoclonal antibodies (mAbs), especially immune checkpoint inhibitors (ICIs), have become essential in cancer treatment. Understanding the PK of these drugs is crucial to optimize dosing for individual patients and improve treatment efficacy. Part I of the thesis focuses on anti-PD-1 PK and explores possible exposure-response relationships. The findings suggest that higher exposure to anti-PD-1 therapy correlates with better treatment response and overall survival. Factors such as body surface area, sex, and albumin influence mAb PK. Part II delves into associations between biomarkers, patient characteristics, and adverse events (AEs) and treatment outcomes during treatment. Levels of granzyme B, immune cell subsets and thyroid toxicity may predict treatment outcomes. The MULTOMAB-trial provided a rich dataset for studying these factors. Future perspectives include standardizing outcome measures and performing prospective trials.
